District 5190 Request for Reimbursement

It is the policy of the District that no money will be disbursed without proper documentation. Please use this form: REIMBURSEMENT FORM, to assist in submitting your receipts for reimbursement. Please check off the proper boxes that relate to your expenses and your position or use the notes area as needed in special circumstances. 

Receipts can be emailed or physically mailed to the District Treasurer for reimbursement. The address for submission can be found on the request for reimbursement form. Feel free to contact the District Treasurer here with any questions or concerns when completing the form.
